A breast lift, or mastopexy, is a cosmetic surgery procedure that raises and reshapes breasts that have sagged from pregnancy, from significant weight changes, or from the natural process of aging. The breast lift restores the breast to a more youthful look by repositioning the nipple at a higher position and removing excess skin. When the remaining skin is tightened, it creates a natural sling–or a new bra–to support the breast.

Dr. Ted Eisenberg, a double board certified Philadelphia cosmetic surgeon, performs breast lift procedures in the safety of the operating room at Philadelphia's Nazareth Hospital. He works with a team of sensitive and competent registered nurses and certified anesthesiologists. The breast lift surgery takes about three hours and is performed under general anesthesia. Patients go home later in the day.

 

Who is the procedure for?

While many women can reclaim their slim waistline after childbirth, no amount of exercise will return their breasts to their pre-pregnancy shape. From pregnancy, breast skin may have stretched and drooped and breast volume may have decreased. Likewise, women who have gained or lost a significant amount of weight may be left with saggy breast tissue. As women age, the skin naturally loses its elasticity, and breasts lose their shape and firmness.

The elastic fibers in your skin are similar to the elasticity of crepe paper: When you pull it a little bit, it bounces back. When you pull it too much, it remains stretched out. The elastic fibers in the skin of the breast have the same response: too much stretching and they don't bounce back.

Women with sagging breasts sometimes tell us that they don't like the feeling of their breasts pulling forward and laying heavily on their stomach. They may be prone to rashes. Sometimes, they can't get the support and lift they need, even with the best bras.

Try the "pencil test" to see if you are a candidate for a breast lift: Place a pencil horizontally along the crease under your breast, where your bra rests. Look in the mirror. Is the position of your nipple (not the areola, the darkened circular area around the nipple) above, at, or below the crease beneath your breast? If it's below, you would definitely need a lift to restore the breast to a more youthful shape.

Are you satisfied with the size of your breasts when they are lifted by your bra? If so, then a breast lift alone may be the right procedure for you.

If you want more breast volume, you are probably a candidate for a combination breast lift and breast augmentation. This is a common option for women, because breast implants alone won't do the job of a lift. In fact, if implants are put behind droopy breasts, it could give the impression that you have four breasts-the two implants higher and your own breasts lower.

If the nipple sits at the crease, it's a borderline situation. Your surgical goals will determine whether you need a breast lift, a breast augmentation, or both.

Things to consider

While a breast lift can create a nicely shaped breast, the scars from the incision will be visible and permanent. The incision is anchor-like in shape; it goes around the nipple, extends from the nipple down to the breast fold and then across the breast fold itself. For some women, the incision will heal to a thin line; for others it may be thicker and more apparent. Healing is usually very good and is helped by our use of dissolvable stitches. (There are no stitches to be removed.)

Sometimes, the breast assumes a boxy shape below the nipple where the skin was tightened.

Mastopexy or a breast lift may not be permanent if your skin tone was poor before the surgery. Your breasts might sag again to some extent, but not nearly to the point at which they were before surgery. A second operation might be required to retighten the skin of the breast.
